New Patients
New Patients Scheduling
The physicians of Cardiology Associates of Gainesville routinely accept new patients. Following are the general types of new patients we accept at CAG:
- Patients with a referral from another physician
- Patients with known cardiovascular disease
- Patients who need cardiovascular clearance for an upcoming surgical procedure
- Patients seeking a second opinion
- Patients in need of cardiovascular testing only

When you call to make your appointment, please make certain you have these items readily available to facilitate the scheduling process:
- The first and last name of your primary care physician
- The name and contact number of other physicians who have treated you for a cardiovascular related issue
- Your insurance information
- If applicable – the date of your surgical procedure
Generally, we are able to schedule new patients within 10 business days of the call. Should you need an appointment sooner than what is offered, please make certain you share this information with our schedulers.
Schedulers are available from 8 a.m. to 5 p.m., and after-hours voicemail is available to our callers.
Your first visit
Prior to your first visit we encourage you to download our new patient forms and provide the requested information. Make certain you bring these with you.
Additionally we ask that you bring ALL medications that you take on a regular basis. This includes over-the-counter medications, dietary supplements, vitamins, and medicines prescribed by any other practitioner. If you have had any lab work done recently, you should also bring this information for the doctor to review.
New patients who have not downloaded and completed the required new patient forms should arrive at the office at least 15 minutes before their assigned appointment. This will allow time for all paperwork to be completed. Those who have completed the work are asked to be here at least 5 minutes before their appointment time.
All new patients must have a form of identification with them and all relevant insurance cards. Cardiology Associates of Gainesville does collect co-pays and deductibles at the time of service. Additional fees may be added if billing service is requested.
New patients for Dr. Silverstein and Dr. Tong
Please note that these physicians ask their patients with appointments before noon to take their routine medications, but to eat or drink nothing else the morning of your appointment so that lab work may be done.
Cancellations
Should you find that you will not be able to keep your appointment or that you will be late, please contact the scheduling office at (352) 264-2516 or (386) 487-0118 immediately.
We will make every effort to accommodate your needed change. You may leave cancellation or change request messages on our after-hours voicemail system.
